To Request a Duplicate Sacrament Certificate:

1. Download, print and complete the form below
   • Parents may request duplicate for children ONLY under 18 years of age.

2. Photocopy a valid California ID
   • Parent or self if person is over 18 year of age.
3. $10 fee required
   • Please make check payable to: Holy Family Church
   • $10 fee applies for EACH duplicate Sacrament request (Baptism, First Communion, Confirmation and Marriage).
4. Process time
   • Takes 5 to 10 days
